Grant Request Guidelines
Priorities
Hawaii Dental Association Foundation’s mission is to improve the oral health of the people of Hawaii by supporting community access to oral care, educational programs, and other charitable community outreach programs. Grants will be limited to projects related to the advancement of oral health. The HDA Foundation will award a maximum of $5,000 per grant. The HDA Foundation will consider programs and projects that promote:
- Oral health education
- Prevention of oral disease
- Access to dental care by underserved populations
Eligibility
To be eligible for funding consideration, an organization must be a Hawaii charity with tax-exempt status under section 501(c)(3) of the Internal Revenue Code.
Grants will not be considered for:
- Multi-year projects
- Individuals
- Scholarships
- Endowments
- Religious programs
- Political organizations or political campaigns
- Organizations with open and active grants with the HDA Foundation
Requests should include:
HDA Foundation Grant Application Cover Sheet (click here for fillable form)
- A brief history and mission of the organization
- A project description stating objective and timeline
- A statement of need or opportunity that this project will address
- The project’s proposed budget & other sources of funding
- A plan to measure the program’s effectiveness
- A plan for recognition of the Foundation
Supporting Documentation:
- Internal Revenue Service determination letter of 501(c)(3) tax-exempt status
- List of current board of directors
- Current operating budget
Application Deadlines
Deadlines to receive requests are May 1 and November 1. Applicants will be notified 60 days after each deadline.
